Allow Duo to be disabled for all

There's currently an option in the admin UI to disable Duo, but only in such a way that it can be overridden by users. Given that Duo requires third-party integration, it would be nice to be able to disable Duo support entirely, such that the Duo 2FA option won't display to users.
