Send custom response on real time changes

Example: I have users listening to lobby, when a the lobby number changes, I would like to do a sql query to get COUNT of lobbies and publish it to all listeners.
Would be super cooler if we can have a custom endpoint that gets executed and its response gets sent instead when a real time is triggered.
